Office Policies

• Please allow up to 48 hours for all prescription refill requests. For faster processing, we recommend contacting your pharmacy first. 
• Prescription refills are not processed on weekends. 
• Please allow up to two weeks for lab results. Results will be e-mailed unless they are urgent, in which case our office will contact you directly. 

Billing & Insurance

• All services will be billed to your insurance. Patients are responsible for understanding their coverage and benefits. 
• Payment is due at the time of service for all self-pay visits. 
• Please bring a valid ID and your current insurance card to each visit. If insurance information is not provided, you will be treated as a self-pay patient and payment will be required at the time of service. Documentation will be provided for you to submit to your insurance for reimbursement. 
• We recommend contacting your insurance provider to confirm that our office is within your network. 
• Motor vehicle accident visits must be paid at the time of service, as we do not bill third parties. 

Appointments & Cancellations

• Patients arriving more than 15 minutes late may be asked to reschedule. 
• We require at least 24 hours' notice for cancellations to avoid a $75 no-show fee. 

Additional Information

• If a minor will be accompanied by someone other than a parent or legal guardian, a completed consent form must be on file.
• In compliance with HIPAA regulations, we are unable to release patient information to anyone other than the patient without proper authorization. 
• Please allow 1–2 weeks for requests of medical records, including immunization records.
