GENERAL OFFICE POLICIES
- For all prescription refills allow 48 hours for refill. This process will go much faster if you contact your pharmacy first.
- No prescriptions will be refilled over the weekend.
- Please allow up to 2 weeks for lab results. Results will be mailed to you unless they are urgent, in which case our office will call you.
- All services will be billed to insurance. You are responsible for knowing what your insurance benefits are and your coverage for those services.
- All self-pay visits must be paid at the time of service.
- Bring your up to date insurance card and ID to your office visit. If you do not have your insurance information you will be considered self-pay and charged at the time of service for the visit. We will give you the appropriate paperwork to submit to your insurance company for reimbursement.
- Please call your insurance company to see if we are in their contracted network.
- All motor vehicle accident appointments must be paid at time of service, as we do not bill third party.
- If you are more than 15 minutes late you may be asked to reschedule.
- Please provide 24 hours notice of cancellations, to avoid a no-show charge of $50.
- If you plan on sending a minor with someone other than a parent, please be sure to have filled out the consent form (under the forms tab).
- Due to HIPAA laws we can not give any information on our patients to anyone other than that patient.
- Please allow 1-2 weeks if you are requesting any copies of your medical records, including copies of immunization records.
- After hours, telephone calls are subject to a $10.00 charge.
